Personal Data Request Form
This form is to support a “verifiable consumer request” for personal data.
That is defined as “a request that is made by a consumer, by a consumer on behalf of the consumer’s minor child, or by a natural person or a person registered with the Secretary of State, authorized by the consumer to act on the consumer’s behalf.
In addition:
You may make a personal data request twice within a 12 month period.
The request must include sufficient information that allows us to verify you are the person we will assess the data request for.
Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond.
